Scheduling an appointment
To schedule your appointment, simply call (574) 287-5859. We offer a wide variety of appointment times to accommodate your schedule. Regular hours are
Monday through Friday 8:00 am to 6:00 pm and Saturday 9:00 am to 12:00 pm. If you require an earlier or later appointment, please advise us and we will
work out an agreeable time.
If you are in severe pain, please advise us when you call and we will schedule an appointment within 24 hours.

Parking is located off the street and within a few feet of the office entrance. Of course, we are wheelchair accessible too.
New Patient Forms
In order to provide better service to our patients, we have made several required forms available for download in Microsoft Word format. If you wish,
you may fill out these forms at home when it is convenient and all the information is readily available. When you are finished, simply print them out
and bring them with you to your first visit. Please choose the appropriate form from the list.
Tarsal Tunnel Syndrome
Tarsal Tunnel Syndrome is a condition that occurs from abnormal pressure on a nerve in the foot. The condition is similar to Carpel Tunnel Syndrome
in the wrist. The condition is somewhat uncommon and can be difficult to diagnose.

Corns, Calluses and Nails
If you require care of long nails, corns or calluses, we will be sure to offer you the option of a relaxing whirlpool treatment prior to your care. We will
also smooth your nails and calluses with an electric file attached to a vacuum so there is no danger of inhaling any nail dust. If you are diabetic or have
high blood pressure, we will be happy to check your blood sugar or blood pressure during your appointment for FREE.
